Penalty Enforcement

What is the correct penalty for the following situations:
(a) Failing to unpile at the end of a down
(b) Prolonged carrying the ball after it becomes dead
(c) Coach-referee conference (to request change in ruling) after all allowed time-outs have been used and no change in a ruling results
(d)Failure to wear required equipment prior to the snap
(e)Snapping the ball prior to the ready for play
(f)Kicking the ball prior to the ready for play
